Individual vs Family Health Plans California 2025

Should you get separate individual plans or one family plan? Understanding how pricing and coverage works for families can help you make the most cost-effective choice.

Overview

In California's individual market, there's no traditional "family rate." Instead:

  • Each person has their own premium based on age
  • Children under 21 have the same rate regardless of age
  • Only the 3 oldest children are charged (4+ children cost the same as 3)
  • Family members can be on the same or different plans
Key Point: A "family plan" in California is really just multiple individual premiums added together. Each family member can choose their own metal tier if desired.

How Family Pricing Works

Example: Family of 4 (Silver Plan)
Family Member Age Monthly Premium
Adult 1 40 $450
Adult 2 38 $420
Child 1 12 $260
Child 2 8 $260
Total Family Premium $1,390/month

Age-Based Pricing Factors

Adults' premiums increase with age. Here are typical ratios compared to a 21-year-old:

  • Age 21: 1.0x (base rate)
  • Age 30: 1.14x
  • Age 40: 1.28x
  • Age 50: 1.79x
  • Age 60: 2.40x
  • Age 64: 3.0x (maximum allowed ratio)

Deductibles & Out-of-Pocket Maximums

Family plans have both individual and family limits:

Limit Type Individual Family (2+ people)
Maximum Out-of-Pocket $9,200 $18,400
How It Works Each person's costs apply only to themselves Costs pooled; once family max is met, plan pays 100% for everyone
How Family Deductibles Work: Each family member's costs count toward both their individual deductible AND the family deductible. Once anyone hits their individual deductible, that person's coverage kicks in. Once the family deductible is met, coverage kicks in for everyone.

Adding Dependents

Who Can Be Added as a Dependent?

  • Spouse: Legal spouse (including same-sex marriage)
  • Domestic partner: Registered domestic partners
  • Children: Up to age 26 (including stepchildren, adopted, foster)
  • Tax dependents: Anyone you claim as a tax dependent

When to Add Dependents

  • Open Enrollment: Add anyone during annual enrollment period
  • Marriage/Domestic Partnership: 60-day SEP to add new spouse/partner
  • Birth/Adoption: 60-day SEP to add new child
  • Loss of Coverage: 60-day SEP if they lose other coverage
